Service Records

Retirees desiring a copy of their discharge certificate (DD214), personnel records or medical/dental records should write to:

All requests must be written, dated, and signed and should include full name, rate/rank, Social Security Number, dates of service and date of birth. Requests for records are processed in four to six months. You may want to visit their Web Site at http://www.archives.gov/index.html prior to making any request.
