Mishap Procedures

MISHAP TRAINING COURSE

SUPERVISORS

  • Send personnel to dispensary
  • Civilian fill out dispensary permit form OPNAV 5100/9
  • Medical Referral Form
  • Military sick call
  • Initiate investigation and call the Safety office by the close of next business day.
  • Supervisor will report all mishaps to the safety office via ESAMS (within 6 days of the incident). 


INVESTIGATIONS
Talk to witnesses and check equipment/material/vehicle involved
Review SOP, training, procedures and guidelines that were followed.
Safety Specialist will investigate further as required.

UPDATES
The Safety office needs notification when there is updated information on an injured employee, i.e. an employee was placed on light duty, but two weeks later was placed on total disability for 7 days.

CLOSE THE LOOP
A case is not considered closed until data entry is complete, any required reports have been submitted in a timely manner, and there is documentation showing that the recommended corrective action has been accomplished, i.e. training rosters, verification of completed work request, SOPs written, etc.

Note: Deaths or incidents that cause 3 or more personnel to be hospitalized are to be reported immediately by supervisor to safety! The Safety Manager shall then immediately report the incident to headquarters. Also report immediately any class A or B mishap and all weight handling equipment mishaps.
